For Referring Providers
Collaborative, comprehensive assessment for your patients.
Stillwaters Assessment & Neuropsychological Services (SWANS) provides psychological and neuropsychological evaluations for children, adolescents, adults, and older adults across Florida. We emphasize clarity, practicality, and partnership—so patients leave with clear answers and actionable recommendations.

- Timely, useful reports: Recommendations tailored to treatment, school, or workplace needs.
- Collaborative care: We welcome communication with treating providers (with a signed ROI).
Common Referral Reasons
We offer evidence-based evaluations addressing a wide range of developmental, cognitive, emotional, and behavioral concerns, including:
- Comprehensive Neuropsychological Evaluations — for concerns involving attention, executive functioning, learning, memory, Autism Spectrum Disorder, neurological or medical conditions, or cognitive decline.
- Psychoeducational Evaluations — comprehensive assessments of learning disorders, academic achievement, and functional educational performance.
- Psychological Evaluations — for emotional, behavioral, and personality concerns.
- Bariatric Surgery Clearance Evaluations — assessing psychological readiness and behavioral factors relevant to surgical success.
- Other Specialized or Comprehensive Evaluations — for complex or co-occurring presentations.
Each evaluation is customized to the patient’s specific concerns, referral questions, and goals.
Please note: SWANS does not conduct forensic or court-ordered evaluations.
Evaluation Process
- Intake (telehealth): Clinical history, context, and referral questions.
- Testing: Telehealth or hybrid administration following validated procedures.
- Record Review: Includes up to 20 pages or 45 minutes of review. Additional time may be billed if needed.
- Feedback: Results, diagnoses (when applicable), recommendations, and next steps.
All telehealth services comply with Florida Rule 64B19-18.004.
Private-Pay Fees
SWANS is a private-pay practice. Evaluation fees generally range from $950–$2,500, depending on the type and scope of evaluation needed.
Focused evaluations addressing a specific referral question typically fall on the lower end of this range, while comprehensive psychological, psychoeducational, or neuropsychological evaluations involving multiple concerns, cognitive testing, diagnostic clarification, and detailed recommendations typically fall on the higher end.
During the intake appointment, we determine the most appropriate evaluation plan and provide the patient with a clear fee estimate before any testing is scheduled.
Additional record review, addenda, letters, or provider consultations may be billed separately when requested or clinically necessary.
Payment Flexibility: Limited accommodations available to improve access.
