We understand that cost is an important consideration when seeking an evaluation. At Stillwaters Assessment & Neuropsychological Services, we strive to provide transparent pricing and individualized recommendations so you can make informed decisions about your care.
Because every person’s needs are different, the cost of an evaluation depends on the type and scope of assessment recommended after your intake appointment.

Evaluation Fees
Evaluation fees typically range from $950–$2,500, depending on the type and scope of evaluation.
Some individuals benefit from a focused evaluation designed to answer a specific question, such as a bariatric surgery psychological clearance evaluation or a targeted diagnostic concern. These evaluations typically fall on the lower end of the fee range.
Others benefit from a comprehensive psychological or neuropsychological evaluation involving multiple concerns, cognitive testing, diagnostic clarification, and detailed recommendations. These evaluations typically fall on the higher end of the fee range.
During the intake appointment, we will discuss your goals, determine the most appropriate evaluation plan, and provide a clear fee estimate before any testing is scheduled.
No testing appointments are scheduled until you have reviewed and agreed to the recommended evaluation plan and associated fees.
Payment Structure
Intake Appointment
- Fee: $300
- Due at the time of scheduling
- Applied toward evaluation fees if you proceed with testing
- Non-refundable
Testing Appointment
- Deposit: $500
- Due when testing is scheduled
- Applied toward your total evaluation fee
- Remaining balance due three days before testing
Feedback Appointment
All evaluation fees must be paid before the feedback session. Reports are released after the feedback session has been completed and all financial obligations have been satisfied.
What’s Included in an Evaluation?
Depending on the type of evaluation recommended, your evaluation may include:
- Comprehensive clinical interview
- Review of relevant records and background information
- Development of an individualized testing plan
- Psychological, neuropsychological, and/or academic testing
- Test scoring and psychometric analysis
- Diagnostic formulation and clinical interpretation
- Comprehensive written report
- Individualized recommendations
- Feedback session to review results and discuss next steps
- Communication with referring providers, when authorized
Evaluations are tailored to the individual and may vary based on referral questions, complexity, and clinical needs.

Why Private Pay?
Stillwaters operates as a private-pay practice because it allows evaluations to be guided by clinical needs rather than insurance requirements.
Private-pay evaluations offer several advantages:
- Greater privacy – Evaluation results are not automatically shared with insurance companies.
- No insurance restrictions – Testing decisions are based on clinical needs rather than coverage limitations.
- Faster access – Evaluations can often be scheduled more quickly without prior authorization requirements.
- Comprehensive assessment – Evaluations can be tailored to your specific concerns and referral questions.
- Detailed reports and recommendations – Reports are designed to provide meaningful answers and practical next steps.
For many individuals seeking diagnostic clarification, accommodations, or treatment planning, a private-pay evaluation provides greater flexibility and a more individualized assessment process.
Insurance & Reimbursement
Stillwaters does not bill insurance directly. However, a superbill can be provided upon request for potential out-of-network reimbursement.
Reimbursement is not guaranteed and depends on your specific insurance plan, deductible, and out-of-network benefits. We encourage you to contact your insurance company directly regarding coverage for psychological and neuropsychological testing.
Additional Services
Most evaluation-related services are included in the evaluation fee. However, services requested after report completion may incur additional fees, including:
- Report addendums or updates
- Additional record review
- Consultation with schools, employers, attorneys, or healthcare providers
- Participation in meetings, such as IEP, 504, or workplace accommodation meetings
- Additional documentation requests
Any additional fees will be discussed with you in advance before services are provided.
